eQuality - Pathways to Potential is an adult day program that servers developmentally disabled adults in Hennepin County. The program services primarily the north and west suburbs including Maple Grove, Plymouth, Brooklyn Park, Brooklyn Center, New Hope, Crystal, Minnetonka and Golden Valley. Using a community-based approach, eQuality offers services including vocational placement and support, educational opportunities and community integration/recreation activities.
Vocational services include assessment, placement in appropriate employment, and staff support sufficient to guarantee success.
eQuality - Pathways to Potential is committed to providing the opportunity
for individuals with developmental disabilities and their families to maximize
their potential and to fully participate in life's opportunities.
In a cooperative environment eQuality - Pathways to Potential will provide opportunities based on the following principles:
Individualism
We will recognize and celebrate the uniqueness of individuals and provide opportunities and experiences based on specific abilities, wants and needs.
Holistic
We will provide opportunities with active program associate involvement based on the recognition of specific dynamics and forces impacting on that person's life.
Respectful
Programs and services will be provided that recognize and celebrate basic human dignity.
Innovative
A flexible approach to individual experiences, which maximize opportunities and best enhance the abilities of program associates, will be the norm. The "Status Quo" will routinely be challenged in an effort to provide the broadest range of experiences possible.
Rights/Responsibility
Program associate choice, based on an informed decision making process, will be the norm. Rights versus responsibilities will represent the basic service delivery tension.
Quality
Excellence will always be the standard in providing programs and services on behalf of program associates. Decisions and approaches will always be made based on providing the best experience possible.
eQuality - Pathways to Potential was founded in 1998 by three individuals with over sixty years experience in non-profit services to the developmentally disabled. Funding is provided through a purchase of services contract with Hennepin County and the program is licensed by the Minnesota Department of Human Services. The organization presents an alternative approach to day services by being totally community based. There is no "home" facility and all activities are based in businesses and community centers. Key relationships have been established with the Maple Grove Community Center, Plymouth Ice Arena, and Plymouth Creek Community Center. The first participant/associate joined in 1999 and the current enrollment is over fifty. There are ten permanent staff members.
